Your form will be sent to your provided address, along with a pre-paid envelope addressed to Hope for a Child. Simply fill out your form and pop it back in the post!

Each form contains a unique personal number and is valid for 2 weeks.

Once we receive your form, you will receive a welcome pack in the post.
